Where Care is Provided

Care Where You Want It:
Our goal is to provide care where the patients live. The majority of patients prefer to remain in their own homes. However, we also care for them in designated Nursing Homes and assisted living facilities, in participating hospitals, as well as our own homelike Hospice Inn in Melville, and our dedicated Hospice Unit at Franklin Medical Center in Valley Stream. Adjustments can be made in living arrangements throughout the course of each patient’s illness, based on patient and/or family needs.
